Carer's self-assessment
Our carer's self-assessment is designed to help you think about your situation and consider what you need support within your role as a carer.
Before you start
You should think about what works well in your life, how you currently manage day to day and what you would like to change. It may be helpful to do this with a family member, friend or someone else you trust.
You can complete the form for yourself, or on behalf of someone else.
If at any point you feel you need help with completing this assessment because you are having difficulty understanding or remembering the information or communicating your views, please contact us on 020 8871 7707.
Once you have submitted the information to us, someone will contact you about the information you have given and discuss what support may be available to help you. We will also talk to you about your emergency arrangements and can answer any questions you may have.

What you will need
Before you complete this form, you will need the following:
- A valid email address if you wish to be sent a copy of your completed evaluation, or to save the form as you go along
- If you are completing this form on behalf of someone else, you will need to know their personal details (this includes their address and contact number)
If you do not understand a question or are concerned that the form does not address your needs, you should contact our Adult Social Care team for support.
What happens next
If you choose to submit your self-assessment to us, we will contact you within five working days to discuss the information you have given.
